Free COVID-19 and Antibody Testing

Clayton Valley Charter High School has partnered with the City of Concord’s Parks and Recreation Department to offer FREE COVID-19 and antibody testing at Centre Concord. You can be tested at this site with or without insurance.

Testing will be offered EVERY Wednesday from 8 a.m. to 4 p.m. through May 26, 2021 at Centre Concord, 5298 Clayton Rd. in Concord.

TO SIGN UP

You MUST schedule an appointment using the online scheduler, which can be found HERE

Appointments open up every Wednesday at 5 p.m. for the following Wednesday.
For example: Appointments for Wednesday, Feb. 24 will become available on Wednesday Feb. 17 at 5 p.m. If no appointments are available, make sure to check back on Wednesdays at 5 p.m.

ON TESTING DAY

You MUST bring:
1. Your Insurance card or a copy of the front and back
OR your Uninsured Patient Form if you are not insured

2. Your completed paperwork, which includes:
- Patient Test Request Form
- Venous Consent form (only if you are requesting antibody testing)

AFTER TESTING

The sample(s) will be sent to the laboratory for processing, and results will be sent to our local health department.
Participants will be able to access results through the PMH Laboratory website after 3 business days.
No further actions will be required if the results are negative.  If the results are positive, a representative from the lab will contact you and a consultation can be scheduled with a lab scientist or doctor to review the results and next steps.
