Care Manager

Meals on Wheels Diablo Region has Care Management Services that can help senior citizens with a variety of issues; housing eviction/foreclosure, family problems, feelings of anger/loneliness, need counseling, health care assistance, legal assistance, and more. Frauds, scams and Alerts 

The pandemic has shown us the best of people, but unfortunately, it also shows the worst. Here are some resources to get the latest in frauds, scams, and other alerts. Protect yourself and be informed.

Fresh Approach’s Mobile Farmers’ Market will also continue to offer service as an essential food resource in the community. Market routes will be adapted to serve one site per city, for a longer period of time at those sites.

The Mobile Farmers’ Market offers fresh, local produce with a 50% discount for all residents who receive CalFresh, WIC, SSI/SSDI, Medi-Cal, food pantry services, or are identified as food-insecure via a short survey.

Emergency Food Distribution at Monument Crisis Center

Monument Crisis Center will continue to hold its emergency food distribution events on Mondays and Tuesdays from 9 a.m. to 12 p.m. this week and next. Please see the flyer or their website for more information: 

Raley’s- Senior Essentials Bay ($20) intended for seniors and at-risk customers in self-isolation or quarantine, team members will build as many Senior Essentials Bags as they can each night and put them in the front of their stores each day at 7 am. They will be available to customers each day depending on inventory levels and while supplies last. Items included in bags will be a mix of fresh items and pantry staples. 

Meals on Wheels Diablo Region has 5 day frozen meal packs for delivery. If you or you know of a senior who is in need of this service, please visit Pickup for Concord Senior Center is on Thursdays. You must register for the program to receive the meals. Please call 925-937-8311.


TeleCare provides free daily reassurance calls to housebound people for the health and well-being of the client as well as a friendly exchange of ideas. Trained volunteers call clients Monday through Friday mornings, beginning at 9 a.m. TeleCare provides a sense of community to clients and the reassurance that someone is checking in with them. If you’re feeling lonely and welcome a friendly chat, TeleCare is here to help.

LGBQ++ Senior Citizens

Rainbow Community Center We have long partnered with the Rainbow Community Center for education and information for the LGBTQ++ community. They have a few programs for seniors in particular. Visit their website if you are interested in finding out more.

Aging with Pride: IDEA(Innovations in Dementia Empowerment and Action), is an innovative program designed to help older adults with memory loss and their care partners (spouse, partner, friend, or others who assists them). To be eligible to participate in this program, either the person with memory loss or their care partner identifies as a member of the LGBTQ community and is age 50 or older. Visit their website: 

Friendship Line

Institute on Aging’s 24-hour toll-free Friendship Line is the only accredited crisis line in the country for people aged 60 years and older, and adults living with disabilities. We also make ongoing outreach calls to lonely older adults. While there are other organizations that respond to the needs of people who may be contemplating suicide, none provides the type of services that IOA’s Friendship Line offers to respond to the public health problem of suicide among the elderly. Knowing that older people do not contact traditional suicide prevention centers on a regular basis even if they are considering suicide, we created the only program nationwide that reaches out to lonely, depressed, isolated, frail and/or suicidal older adults. Our trained volunteers specialize in offering a caring ear and having a friendly conversation with depressed older adults.

Contra Costa County has said that testing is increasing. You should check with your provider, but if they don’t have testing you can call their community testing line for a drive-through testing appointment.

Legal Services

Contra Costa Senior Legal Services provides senior citizens with legal advice, housing help, elder abuse, consumer protection, advanced health care directives, durable power of attorney, and other resources. Visit or call 925-609-7900.

Contra Costa Bar Associations offers a few resources for the community as well; Find an attorney, immigration (En Espanol), conflict resolution, rent/housing resources, and more. Visit or call (925) 825-5700. They have an online request form you can fill out as well.

Contra Costa County HICAP Counseling

Registered HICAP counselors provide free assistance for Medicare-related matters in person or by phone. (Counseling will be done via phone)
