Bed Bug Complaint

  1. Notice to Landlord Confirmation*

    In order to submit a bed bug complaint to the City, you must have first notified your landlord in writing about the presence of beg bugs and requested treatment. If you have done this and the landlord failed or refused to provide treatment, or the treatment has not resolved the issue, then check the box below and proceed submitting this complaint form.

  5. The address, including unit #, must be provided so that we can ensure treatment occurs in the correct unit. You do not need to enter the City or State

  6. A staff member from the Multi-Family Inspection Program will contact your landlord to ensure treatment takes place. After proof of treatment is provided from the landlord to the City, we will contact you to confirm treatment was successful.
